Self-referral

You may refer your self for physiotherapy assessment - simply print this form and then send/ hand in your form to the one of our physiotherapy departments.

This service is only for adult out-patient physiotherapy assessment. Usually our clinics aim to offer you an appointment within 2 weeks. However, since we launched the "self-referral" service many more people are requesting physiotherapy. As a result of these increased numbers, you may have to wait longer for an appointment. Clinics will offer you an appointment as soon as they are able to.

Collars, splints etc are only loaned in conjunction with rehabilitation and therapy. Long term provision, or replacement of such items cannot be provided. Self-referral to physiotherapy for the simple provision of walking aids (including wheeled frames, crutches, sticks etc.) will not be permitted.

Home visits can only be arranged by your GP or other healthcare professional.

If your problem is severe, or getting worse, please see your GP or seek more urgent medical attention.
